SINGAPORE: Some social workers have said many senior citizens are not aware of the government's policies for the elderly. But one seniors activity centre in Sin Ming has come up with a novel way to keep track of senior citizens under its charge and update them on the resources available. Social workers at Comnet@Sin Ming visit their charges regularly to explain the centre's activities and programmes and to get to know new residents. They make a note of the residents' mental and physical condition to make sure more attention is paid to those with health problems. After each visit, they make notes on a white board to update one another. Social workers said this helps make sure no one falls through the cracks. Comnet@Sin Ming programme coordinator Lim Yu Bing said: "Through home visits, we got to know about their condition, and also whether they need any assistance. "It's all through these home visits that we got to know what kind of help they need, then we can link them to the different community resources. Through these community resources, they will be able to get the help they need."
